List of Items to Bring to an Appointment
Payment is due at the time of visit for all outstanding charges, co-pay, deductible and coinsurance. We except checks, cash, and credit card. For all credit card payments, there is a 3% convenience charge which is the amount the credit card companies charge for processing payment.
Please bring all your insurance cards to each appointment. If you have more than one insurance plan, you will need to provide us with all the insurance plans detail. Failure to do so will result in rejection and delay of the reimbursement from your insurance company.
Any Authorizations, pre-certifications, or referrals must be obtained before your visit.
Children brought in by friends all family members who are not legal guardians will need written consent and verbal confirmation by FaceTime that the child can be seen. Please understand that it is illegal for us to provide nonemergency care to a child without the permission of the legal guardian or parent.
Bring all pertinent records from other physicians, hospital summaries, laboratory results and x-rays that relate to your appointment.
It would save you a lot of time if you can prefill and sign the forms before the visit. This is especially important for new patients.
Driver's license, insurance card, and legal custody papers- please bring these along to the visit.
Please copy the names of the medications and the exact dose of medication that your child is taking. If in doubt, bring the bottles.
Every patient will get a school excuse note for the date that they are seen in the office. We cannot provide retroactive excuses for your child if they were not seen in the office on the day they were absent. Please do not bring a list of dates that they were absent from school on your visit expecting that the physician will excuse your child. However after a diagnosis has been made, we can write to you of school explaining that there is a medical condition that may require certain accommodations in future.
FMLA, work-related forms of any sort, or college-related forms—there is no charge for this.
